What is the Case Management Program?
The Polaris Case Management Program is an outcomes-focused case management system designed to help clients and their loved ones navigate critical transition periods, including early recovery. Case management can also provide the long-term support needed by some individuals to maintain the highest quality of life in the least restrictive environment. Each program is individually tailored to fit the unique needs of each client, providing dedicated clinical support and life skill coaching to build confidence and independence. With a proven track record of improving recovery rates, our comprehensive aftercare program creates a custom-developed path for each client, combining clinical case management with practical coaching services to guide both clients and their family members. This support is imperative through the particularly vulnerable period between inpatient treatment and fully independent living and can continue to be useful in long-term maintenance and relapse prevention.

Sober Companion
Sober companion work involves providing one-on-one support to individuals in early recovery from substance use disorder. Our sober companions accompany and support clients during high-risk periods, such as immediately following treatment or during transitions back to daily life. Their role is to help the client maintain sobriety by offering structure, accountability, and real-time guidance, often living with or spending extended hours with the client. Sober companions model healthy behavior, assist with daily routines, encourage participation in recovery activities, and help the individual navigate triggers and stressors. The goal is to create a stable environment that fosters long-term recovery, offering both emotional support and practical tools to reduce the risk of relapse.
Vocational Coaching
Vocational coaching helps individuals turn progress in recovery into real-world stability. Through personalized support, we assist clients in identifying strengths, setting goals, and securing meaningful employment. Services include resume building, interview preparation, job placement guidance, and ongoing accountability. By fostering structure, confidence, and independence, vocational coaching becomes a key component in building a stable and purpose-driven life.
